Plan Features
Global Coverage – Coverage is worldwide including the United States.
Two Plan Options – Applicants can choose from a high level of coverage with the CitizenSecure or budget styled plan with the CitizenSecure Economy.
Optional U.S.A. Coverage – The CitizenSecure can be purchased with or without U.S. coverage. ITG Worldwide highly recommends that U.S. expatriates apply for U.S. coverage unless they are covered by Medicare or a self-insured corporate plan.
Preexisting Condition Coverage – CitizenSecure offers coverage the same as any illness or accident provided the condition was disclosed on the application and not excluded or limited by rider. The Citizen Economy offers up to $5,000 per certificate period to a lifetime maximum of $50,000 after 24 months of coverage.
Lifetime Coverage – Members enrolled in the CitizenSecure plan by their 65th birthday and maintaining continuous coverage to age 75 are eligible for lifetime coverage. Prior to the members' 75th birthday, they may elect to enroll in the CitizenSecure Senior Plan ® for lifetime coverage. No medical underwriting is required. Members must continue to meet the eligibility requirements.
CitizenSecureSM Assistance Services – All Assistance Services are available 24/7 and include pre-trip health and safety advisories, bag track and emergency cash transfers. Please see the Providers and Assistance section for a complete list of services.

Carrier Information
HCC Medical Insurance Services (HCCMIS), established in 1998, is a full service insurance organization offering a comprehensive portfolio of international health and travel insurance products for groups and individuals. HCCMIS’s team of claim specialists, medical professionals and client relation specialists has assisted tens of thousands of clients.
HCC Medical Insurance Services was acquired by HCC Insurance Holdings, Inc. (NYSE: HCC) in January of 2008. HCC is based in Houston, TX and is a leading international specialty insurance group with offices across the U.S., Bermuda, Spain, Ireland and the United Kingdom. HCC has assets of more than $8 billion. The company receives a rating of AA (very strong) from Standard & Poor’s, AA (very strong) from Fitch Ratings and A+ (superior) from A.M. Best Company.
The CitizenSecure and CitizenSecure Economy plans are insured by Lloyd’s, the largest and oldest market in the world. Lloyd’s receives high marks from independent rating firms AM Best (‘A’, excellent) and Standard and Poor’s (‘A’, strong). Lloyd’s provides accident and health insurance to millions of individuals in almost every country in the world.

Why Do I Need International Coverage?
If you are intending to reside overseas or planning a trip longer than six months, we encourage you to review your current health insurance policy for key residency and overseas coverage clauses. Ask your benefits specialist or insurance agent to clarify if necessary. These clauses typically apply to both individual and group health plans.
- Residency Clause – The primary function of a residency clause is to ensure people live in the state where they are purchasing the policy. Example: A resident of New York may not purchase insurance in Wyoming. These clauses typically require the member to be a resident at least 6 months during a 12-month policy period. If the residency clause is violated, the policy may be void and the insurance company no longer responsible for paying claims.
For American expatriates and long-term travelers, this creates a serious problem. International health insurance plans avoid this problem by allowing the member to reside outside the U.S. for indefinite periods of time.
- Coverage Outside the U.S. with Your Current U.S. Health Plan - The majority of U.S. health plans offer emergency care only when outside the U.S. In addition to this dramatic reduction in benefits, most U.S. health insurance companies are not able to assist members outside the U.S. It will be your responsibility to arrange care if you have a medical emergency and require medical evacuation or access to a doctor or hospital.
The advantage and purpose of purchasing travel medical and international health insurance is to avoid placing the burden of care on the member by offering services such as, 24/7 multilingual emergency assistance, networks of overseas providers and the ability to navigate foreign healthcare systems.
Benefits Summary
HCC Medical Insurance Services® offers the CitizenSecureSM plan in two different coverage options, CitizenSecureSM and CitizenSecure EconomySM , to meet a wide range of expatriate health insurance needs. Plan Description
Below are brief descriptions of the benefits for the CitizenSecureSM and CitizenSecure EconomySM plan options.
CitizenSecure Economy – Basic Coverage
The Economy offers members quality benefits for an inexpensive premium. It is a Scheduled Benefits plan, which means it offers specific benefit amounts for specific services. Plan highlights include:

CitizenSecure – High Level of Coverage
CitizenSecureSM is a comprehensive international medical insurance plan. The majority of benefits are not subject to individual maximums with most benefits paying up to the policy maximum according to Usual, Reasonable and Customary charges.
Includes all benefits of the CitizenSecure EconomySM plan plus:
- Coverage Area Choices - option for coverage with or without coverage in the U.S. and Canada.
- Higher Level Coverage - most benefits levels increased to the policy maximum ($5M).
- Preexisting Conditions - covered as any other injury or illness if disclosed on application and not excluded or limited by rider.
- Human Organ and Tissue Transplants - same as any other illness for covered transplants.

Optional Benefits
The CitizenSecureSM plan can be customized to fit your needs by adding optional benefits. These benefits are subject to underwriting approval and require additional premium. Please see the brochure for more details about optional benefits.
Dental
The Optional Dental Rider provides important benefits for families including preventative care for children and basic and major dental benefits for both children and adults. Preventative benefits include routine oral exams and x-rays, cleaning and fluoride treatments. Basic benefits include Periodontics, Endodontics, extractions and fillings. Major benefits include crown, bridges and dentures. Benefit amounts increase with each certificate period.
Term Life and AD&D Insurance
Expatriates and international travelers typically find it difficult to purchase life insurance outside of the United States. HCCMIS makes it easy by offering life and AD&D insurance with CitizenSecureSM in amounts ranging from $5,000 to $50,000 depending upon your age. For those seeking a higher level of coverage this benefit can be doubled.
Sports Rider
Many hazardous sports such as mountaineering, contact sports and professional sports are excluded by the CitizenSecure. The Sports Rider adds up to $25,000 of coverage for extreme sports and up to $5,000 for contact sports.

Global Physician Networks
International Providers
HCCMIS maintains relationships with hospitals and doctors throughout the world, and this network is growing daily. Many of your provider selection decisions will be based on geographical location and provider specialties. HCCMIS will help you find a provider that suits your needs and can work with us through direct billing to save you time and money. The network is located in Client Zone, which is easily accessible with your Certificate number and date of birth once coverage has been approved. You may also contact HCCMIS directly for assistance.
U.S. In-Network Providers – Hygeia
HCC Medical Insurance Services® participates in the Hygeia Network. The Hygeia Network offers services at over 4,000 medical facilities and 151 Centers of Excellence with over 400,000 physicians. The Hygeia Network offers nationwide coverage across the U.S. with concentrations in metropolitan and urban cities, traditional travel states, and many rural areas. The Coinsurance will be waived, expenses are incurred within the PPO Network and payment is made directly to the provider.
U.S. Out-of-Network Providers
HCC members may seek care outside the provider network but should be aware they may incur additional expenses as out-of-network providers are not contracted with HCCMIS. For example, the full deductible and co-insurance will apply. Also, costs exceeding HCCMIS’s UCR charges will be the member’s responsibility.

Pre-certification
CitizenSecureSM plans require pre-certification, which simply means you must contact HCC Medical Insurance Services® as soon as possible before a planned hospitalization or surgical procedure, within 48 hours of an emergency hospital admission or the first 90 days of pregnancy. Pre-certification allows HCCMIS to establish contact and make payment arrangements with your providers, and negotiate discounts benefiting you and HCCMIS.
The following treatments and services must be pre-certified or certain reductions of benefits may result. Check with HCC Medical Insurance Services® for a complete and up to date list of conditions that require pre-certification.

All Assistance Services are available 24/7 to CitizenSecureSM members. Services Include:
Pre-Trip Health and Safety Advisories – You can call HCCMIS for current passport, visa, inoculation and vaccine requirements, as well as up-to-date travel and safety advisories.
Livetravel Services – HCCMIS will make emergency travel and itinerary changes for you including rebooking flights, hotel reservations and ground transportation arrangements.
BagTrak – HCCMIS is the industry leader in tracking lost checked baggage. HCCMIS will help locate lost checked baggage and deliver it to you anywhere in the world.
Emergency Message Relay – HCCMIS will relay messages to your family, friends and co-workers, helping you to maintain contact during an emergency.
Emergency Cash Transfers – HCCMIS will assist you in arranging and obtaining cash transfers anywhere in the world.

Eligibility
General Eligibility
CitizenSecureSM plans are available to citizens of all countries of the world. Applicants must be 14 days old and no older than 74. Non-U.S. citizens may reside anywhere in the world including their home countries. Non-U.S. citizens residing in the U.S. may be ineligible if they qualify for a U.S. health insurance plan.
U.S. citizens must reside outside the U.S. for at least 6 months a year or be departing the U.S. within 30 days of the effective date. U.S. citizens must also continue to reside outside the U.S. for 6 months out of the certificate period to renew coverage. In addition, members must depart the U.S. and Canada within 30 days of the effective date for renewing policies.

Eligible Dependents
Eligible families will receive free coverage for the first two eligible dependent children between the ages of 14 days to 9 years when both parents are insured under the CitizenSecure plan. All family members that are requesting to be covered need to answer the questions on the application.

Important Note
All applications are subject to Underwriting. Eligibility does not guarantee the application will be accepted and a policy issued. Do not terminate any existing insurance until receipt of written approval from HCC Medical Insurance Services®.
Claims
HCC Medical Insurance Services® has developed many tools and services to assist CitizenSecure members’ quick access to care and payment of claims. In many cases, HCCMIS can pay claims directly to hospitals and doctors.

Filing Claims
If you need to be reimbursed for medical expenses, complete the claim form and submit your original itemized bills and paid receipts within 90 days to HCCMIS. Do not hold your bills or receipts until the end of the year. Mail the completed claim form and all required information to:

How to Apply
HCCMIS’s CitizenSecureSM international health insurance plan can be applied for either online or by downloading an application from the link below. Either method is straightforward. However you choose to apply, please provide accurate and current information. It is also very important that you complete the entire application. Unanswered questions will slow the process.
Current Coverage – Please do not terminate any current health insurance coverage until you have received written confirmation from HCC Medical Insurance Services® that your coverage is effective and you accept HCCMIS’s terms and conditions.

The Underwriting Process
HTH’s underwriters will review applications for health history, eligibility and completeness. Before making a final decision, the underwriters may require additional information such as medical records.
Once the process is complete, the underwriters will issue one of four outcomes;

Approved as applied for;
-
Approved for a plan with lower benefits;
-
Approved with a rider excluding or limiting coverage for a specific health condition; or
-
Declined.
ITG Worldwide works hard to make sure applicants receive fair and equitable treatment in the underwriting process. Ultimately, it is HCCMIS’s final decision on whether or not applications are approved.
Please allow at least 5 business days while a decision is reached or a request for additional information is made.
